Pay Scale: $38.01 - $49.93
Shift: Monday - Friday, Days with weekend rotation
Job Summary:
Under the direction of the assigned Therapy Supervisor, the Lead Therapist is responsible to assist provision and coordination of a high quality, client-oriented therapy rehabilitation program that is broad-based in treatment design and implementation. Responsibilities will include coordinating and reviewing case management, training and orientation of staff, monitoring daily scheduling, timecard editing, and involvement in the quality assurance process. Provides direct patient care in compliance with his/her licensure at 55% productivity or greater.
Essential Functions:
Interviews patient, family, caregivers, significant others to obtain information about prior level of function, home environment, family relationships, health and psychosocial history.
Performs client tests, measurements and assessments as appropriate. Plans, prepares, and administers written treatment programs based on evaluation, best practice, and any appropriate age or diagnosis related guidelines.
Establishes measurable/functional treatment goals that are realistic and consistent with patient/family/caregiver needs.
Monitors all patients under his/her care. Supervises Physical Therapist Assistants/Certified Occupational Therapy Assistants and/or Technicians, as applicable per licensure, who are involved with treatment and assures appropriate treatment and documentation. Works with physician as needed for modifications to the plan of care. Implements and assesses appropriate patient and caregiver education per Plan of Care and phases of progress. Assists in planning for the discharge needs of the patient.
Institutes or assists with emergency measures for sudden adverse developments such as cardiac arrest or medical condition requiring activation of a code, rapid response team, or activation of 911.
Maintains timely and appropriate documentation. Meets facility documentation standards as per documentation review process. Enters charges accurately into Meditech or on appropriate charge slip.
Researches, organizes, and presents patient/service/specialty information needed to ensure safe, cost effective, clinically appropriate therapy guidelines for patients, including specialty areas.
Coordinates as well as participates in case management through team conferences. Reviews patient programs as appropriate, discussing progress, plan, and by eliciting/giving feedback with the team.
Orients and trains therapy staff regarding patient care, safety, policy and procedures, and professional behavior upon hire and as needed thereafter.
Coordinates and provides feedback on chart audits. Provides feedback to supervisor for staff annual performance appraisals. Assists with clinical issues and follow up with corrective action plan adherence when appropriate.
Fosters collaboration within the department as well as interdepartmentally. Seeks assistance from supervisor or other appropriate staff members when there are scheduling conflicts, patient difficulties, staff issues or other challenges.
Serves as a resource/mentor for staff related to clinical practice, documentation, and other department processes.
Implements, upgrades, maintains and supports application systems in the department. Acts as a resource to staff and troubleshoots system issues. Provides training and documentation to staff. Develops and maintains department downtime procedures.','!*!License/Certification:
Physical Therapist, Occupational Therapist OR Speech Language Pathology-CCC program licensed to practice in the State of Oregon.
BLS is required upon hire.
Experience:
Three (3) years of patient care experience in a broad based rehabilitation facility is required with the most current year being at AIM Therapies.
Leadership experience or strong leadership skills related to significant project management or team activities in the last two (2) years is required.
Informatics experience is preferred.
Must have strong interpersonal and communication skills, both verbal and in writing.
Education:
